Skin Is Not “Just Skin”

Most people think of skin as a simple covering.

Skin is the largest organ in the human body. It protects us from infection, helps regulate temperature, allows us to feel touch, pressure, heat, and cold, and acts as a remarkable barrier between our bodies and the outside world.

An average adult carries nearly two square metres of skin.

Yet not all skin is the same.

The skin of the eyelid is among the thinnest on the body. The skin of the back is thicker and stronger. The scalp has an excellent blood supply but very little flexibility. The skin on the lower leg often heals differently from skin on the face.

Plastic surgeons spend years learning these differences because reconstruction depends upon them.

The body constantly reminds us that tissue has its own character.

Different Tissues, Different Rules

One of the reasons reconstructive surgeries are so fascinating is that different tissues behave in different ways.

Fat has a relatively delicate blood supply and can be vulnerable after injury or surgery.

Muscle is rich in blood vessels and often heals well.

Scar tissue may be strong, but it is usually less flexible than normal tissue.

Bone heals differently from skin. Nerves heal differently from muscle.

There is no universal rule that applies to every tissue.

This is why reconstruction is not simply about closing a wound. Plastic surgeons must understand what lies beneath the surface and how each layer will respond over time.

Much like a gardener learns which plants thrive in shade and which require sunlight, plastic surgeons learn the preferences and limitations of different tissues.

Success often comes from working with biology rather than against it.

Blood Supply: The Invisible Hero

If there is one factor that determines success more than almost any other, it is blood supply.

Patients rarely think about blood vessels during healing. Plastic surgeons think about them constantly.

Every skin graft, skin flap, scar revision, breast reconstruction, and wound closure depends on blood flow.

Blood delivers oxygen, nutrients, infection-fighting cells, and the building blocks required for healing.

Without an adequate blood supply, wounds struggle.

This is why plastic surgeons handle tissue gently. It is why tension matters. This is why smoking can have such a profound effect on healing.

Interestingly, the tiny blood vessels that determine success are often invisible to the naked eye. Yet these microscopic structures frequently decide whether tissue survives and heals well.

The most important things are not always the most obvious.

Life often teaches the same lesson.

Why Some Wounds Heal Beautifully

Patients often compare their healing to someone else’s.

It is understandable. We all want reassurance.

Yet healing is deeply individual.

Age, circulation, genetics, nutrition, smoking, diabetes, medications, stress, and even the location of a wound all influence the outcome.

Two patients may undergo similar procedures and heal differently.

This is not failure. It is biology.

One of the Stoic philosophers, Epictetus, taught that frustration often arises when we expect outcomes that lie beyond our control.

Healing reminds us of this truth.

We can create the best possible conditions for recovery. We cannot force tissue to heal on our preferred schedule.

The body works according to its own timeline.

The Patient Is Part of the Reconstruction

One of the most important messages I share with patients is that they are not passengers in the reconstructive process.

They are part of the team.

Successful reconstruction does not end when the operation is completed.

The choices patients make afterwards matter greatly.

Elevation helps reduce swelling.

Good nutrition supports healing.

Limiting movement protects delicate repairs.

Avoiding smoking improves blood supply.

Attending follow-up appointments allows problems to be recognised early.

Sometimes the hardest part of recovery is not the surgery itself, but the patience required afterwards.

Many patients feel well before their tissues are fully healed. This can create a temptation to do too much, too soon.

Yet tissue has no interest in our schedules.

Healing requires respect for biology.

The long view matters.

Healing Is a Team Sport

Patients are not alone on this journey.

Plastic surgeons guide the reconstruction. Anaesthetists provide safety and stability during surgery. Wound care sisters help monitor healing, provide reassurance, and recognise early concerns before they become significant problems.

Each member of the team contributes something important.

No one succeeds alone.

Perhaps this is another lesson that extends beyond medicine. The strongest recoveries, like many worthwhile achievements in life, are often supported by a community of people working together toward a shared goal.

Stillness, Skill, and the Long View

Our theme this year has been Stillness, Skill, and The Long View.

Few subjects illustrate these ideas more clearly than tissue healing.

Stillness allows us to observe rather than rush.

Skill allows us to create the best conditions for success.

The long view reminds us that meaningful healing takes time.

As plastic surgeons, we learn early that tissue cannot be bullied, hurried, or negotiated with. It responds to biology and rewards patience.

Perhaps there is a broader lesson here for all of us.

Not everything in life can be forced. Some things require careful attention, consistent effort, and trust in the process.
